Redesignating an Alternate Level of Care (ALC) Patient

A patient can be re-designated ALC only if the discontinue reason was due to ‘Change in Medical Status” and if the re-designation happens within 40 weekdays of the discontinuation date. After the 40 weekday threshold, the patient cannot be re-designated ALC and a new waitlist entry will need to be created.

To re-designate a patient as ALC:

  1. Use the Patient Search link to find a patient.
Note: A Patient's open waitlists can also be accessed directly from the Surgery, DI and ALC Home pages.
  1. Click the View Waitlist Entries icon  from the Patient Search Results screen.
    OR

    Select the patient's name to view the patient's demographics and then click the View Waitlist Entries button.
  2. A list of the patient’s waitlist entries opens. Click the Waitlist Entry ID number to open the required waitlist entry.
  1. Click the Re-designate button and make the required changes.

  2. Enter the appropriate date for ALC Re-designation Date.

Important: When you re-designate, you need to re-populate the following fields:

  • Visit Number (mandatory for Complex facilities, highly recommended for Basic)
  • ALC Discharge Destination Type
  • ALC Discharge Destination Detail (if applicable)
  • ALC Discharge Destination Date
  • Most Appropriate Discharge Destination Type
  • Most Appropriate Discharge Destination Detail (if applicable)
  • Most Appropriate Discharge Destination Determination Date
  1. Click the Save button.
Important: Multiple discontinuations and re-designations may be applied to the same waitlist entry when a waitlist entry is discontinued due to a “Change in Medical Status” and the patient is re-designated ALC within 40 weekdays.

Minimum fields required to re-designate a patient ALC:

  • ALC Re-designation Date
  • Most Appropriate Discharge Destination Type (must be re-selected)
  • ALC Discharge Destination Type (must be re-selected)
  • Most Appropriate Discharge Destination Detail (if applicable)
  • ALC Discharge Destination Determination Date
  • Most Appropriate Discharge Destination Determination Date
